Who we are and what this policy covers
This website, conscious.health, is operated by Conscious.Health Management, LLC ("Conscious Health," "we," "us"), a Texas limited liability company that provides administrative and management services. Medical care is provided by Conscious.Health Medical PLLC (the "Practice"), an independently owned Texas medical practice.
This policy describes how we collect, use, and share information through this website, our membership application, our scheduling tools, and non-clinical communications such as email and text messages.
If you become a patient of the Practice, health information created or received in the course of your care is protected health information under HIPAA and is governed by the Practice's Notice of Privacy Practices at conscious.health/hipaa, not by this policy. Where the two overlap, the Notice of Privacy Practices governs your health information.
Information we collect
Information you provide.
- Contact details: name, email address, phone number, and state of residence.
- Membership application responses, which may include information about your health, medications, and goals.
- Scheduling information when you book a call.
- Messages you send us by email, text, or through the site.
Payment information. Payments are processed by our payment processors. We do not store complete card numbers on our systems.
Information collected automatically. IP address, device and browser type, pages visited, referring URLs, and similar data collected through cookies and related technologies (see Cookies and tracking, below).
How we use information
- To review and respond to membership applications and connect applicants with the Practice.
- To schedule and conduct calls you request.
- To send service communications about your application, appointments, and account.
- To send updates and marketing you have opted into. You can opt out at any time.
- To operate, secure, and improve the website.
- To comply with legal obligations and enforce our Terms of Use.
Text messaging
If you opt in to text messages from Conscious Health:
- Message frequency varies. Message and data rates may apply.
- Reply STOP at any time to opt out. Reply HELP for help, or contact support@conscious.health.
- Consent to receive text messages is not a condition of purchasing any goods or services.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Sharing with subcontractors that support our messaging program — for example, customer service and the platform that delivers our messages — is permitted. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
How we share information
The Practice. When you apply for membership or request care, we share your application and contact information with Conscious.Health Medical PLLC.
Service providers. Companies that support our operations — practice management and electronic records, communications and CRM platforms, scheduling, payment processing, email delivery, and analytics — under contracts that limit how they may use your information.
Professional advisors, such as lawyers and accountants.
Legal. Where required by law or legal process, or to protect rights, safety, and property.
Business transfers. In connection with a merger, acquisition, financing, or sale of assets, subject to this policy.
We do not sell personal information, and we do not share personal information for cross-context behavioral advertising.
Health-related and sensitive information
Your membership application may include health-related information. We treat it as sensitive data: we process it only with your consent, use it only to evaluate your application and connect you with the Practice, and never use it for advertising. Pages of this site that collect health-related information do not carry third-party advertising trackers.
Cookies and tracking
We use essential cookies (required for the site to function) and analytics cookies (to understand how the site is used). You can control cookies through your browser settings. Where required by law, we treat Global Privacy Control signals as a valid opt-out of targeted advertising.
Your privacy rights
If you are a Texas resident, the Texas Data Privacy and Security Act gives you the right to:
- Confirm whether we process your personal data, and access it.
- Correct inaccuracies.
- Delete personal data you provided or that we obtained about you.
- Obtain a portable copy of your data.
- Opt out of targeted advertising, the sale of personal data, and profiling in furtherance of decisions that produce legal or similarly significant effects.
To exercise these rights, email support@conscious.health. We will respond within 45 days; where reasonably necessary we may extend once by 45 days, and we will tell you if we do. If we decline your request, we will explain why, and you may appeal by replying to our response with "Appeal." We will respond to appeals within 60 days; if your appeal is denied, we will provide you a way to submit a complaint to the Texas Attorney General.
Residents of other states may have similar rights under their state's laws and may submit requests through the same channel.
Data retention
We keep personal information as long as needed for the purposes described in this policy, to comply with legal obligations, and to resolve disputes — then delete or de-identify it.
Security
We use administrative, technical, and physical safeguards designed to protect personal information. No method of transmission or storage is completely secure.
Minors
This website and our services are for adults. We do not knowingly collect personal information from anyone under 18.
Changes to this policy
We will post updates here with a new effective date. If changes are material, we will provide notice.
